This form is used for complaints against, or commendations of, a police officer or other police department staff.

Or do you want to commend or recognize an officer/staff member for their actions?
All complaints/commendations will be reviewed. Contact information will allow for follow-up.
Upon filing this complaint/commendation, you should be taken to a page that says "Receipt of your message is acknowledged". If you are not taken to the acknowledgement page, then assume non-receipt and notify the police department at 978.542.6542 or by email at police@salemstate.edu.
The university police department will make every effort to complete the investigation within 30 days and you will be informed of the result of the investigation and the disposition of the complaint within thirty (30) days, unless otherwise advised that more time is required. By filing a complaint, I agree that to the best of my knowledge, the below statement is a true and accurate account of events. I further agree to cooperate with the investigation which may include participating in follow up interview and testifying at any internal or criminal hearing related to this matter.
