Disability Services is introducing Accommodate – an accommodation management system that helps students easily interact with our office.
Students can use Accommodate to register with our office, request and view accommodations each semester, and upload their documentation! Students will also be using this platform to make alternative testing requests.
If you are connecting with us for the first time, please fill out our online intake form. You will need your Salem State-issued email address (example: firstname.lastname@example.org) as well as your SSU ID number, without the S.
If you have documentation in an electronic format, you can submit it on this form. You may also bring your documentation to our office to be scanned. If you are unsure what you need to provide for documentation, please refer to our documentation guidelines.
Once you have submitted the intake form, please reach out to our office regarding scheduling an appointment to discuss your requests. In order to register with our office and receive accommodations, you must meet with a counselor. Through the interactive process, Disability Services will determine what types of services or reasonable accommodations are appropriate. Accommodations are determined on an individual basis.
