Accepted Students Health Information

Congratulations on your acceptance to Salem State College! All accepted students are required to submit immunization and health information before the semester starts. All information will remain confidential and be used for health care purposes only. Copies of forms are below.

• We recommend you obtain a complete medical exam
• Complete the Student Health History before seeing your health care provider.
• Your health care provider must complete & sign Provider Health Form, including the immunization record as required by law.
• The meningococcal vaccine is required for all students living in the residence halls.
• Return the completed forms to SSC Counseling & Health Services
• You can't register for classes until all your health info has been received
• Health Insurance is required by state law. You must either enroll in or waive the SSC Student Health Insurance.
