Information for New Students

Congratulations on your acceptance to Salem State! Before you start classes, you need to make sure the following health insurance requirements are met and health information forms are on file with us.

Health Insurance
Under Massachusetts state law, comprehensive health insurance is required for all students registered for nine or more credits. If you will not have health insurance through an employer or family member, you will need to purchase the Salem State University Student Health Insurance.

Immunization and Health Information
All accepted students are required to submit immunization and health information before the semester starts. Immunization documents are required by state law for college students in order to register for classes. Health information will remain confidential and be used for health care purposes only. Forms are available below.

Health Forms Deadline: Health Forms must be received in Counseling and Health Services two weeks before classes begin.

Part 1 of the Health Form should be completed by the student, parent or guardian.

Part 2 contains the Immunization Record which must be completed and signed by a health care provider. We strongly recommend that you have a physical examination before starting classes. Please check with the department of your chosen major to find out if a physical exam is required.  

The meningococcal vaccine is required for all students living in the residence halls. Please read the information on meningococcal disease and vaccine. Students may waive the vaccination by signing the meningitis vaccine waiver, although we recommend that you obtain the vaccination.

Return the completed forms to Counseling and Health Services. Do not include forms meant for other university departments and offices.

If the Health & Immunization Form has not been received by the second week of classes each semester a hold will be placed on your registration.
