Information for New Students

Congratulations on your acceptance to Salem State! Before you start classes, you need to make sure the following health insurance requirements are met and health information forms are on file with us.

Health Insurance

Under Massachusetts state law, comprehensive health insurance is required for all students registered for nine or more credits. If you will not have health insurance through an employer or family member, you will need to purchase the Salem State University student health insurance.

Immunization and Health Information Policy

The Counseling and Health Services Immunization Form [PDF] must be completed and signed by a health care provider for all entering students.

In accordance with Massachusetts College Immunization Law, 105 CMR 220.600, Salem State University requires verification of immunity against certain illnesses. Exact dates are required for all immunizations and/or serologic test results. If serology titers indicate lack of immunity, vaccines must be administered. If if the Salem State Health Form is not submitted, any documentation presented must meet the guidelines set forth in the "Salem State University counseling and health services immunization form" for compliance.

The student is responsible for verifying immunization compliance. Counseling and health services is not able to individually notify students of compliance status. Information submitted becomes the property of Salem State University. It is recommended that copies of original documentation be submitted and it is the student's responsibility to make and submit the copies.

Registration for classes will be affected if information is not complete. A registration hold will be placed for non-compliance and will remain in place until proper documentation is submitted and meets the state law requirements. The hold may take up to 48 hours to be removed and registration process to be allowed.

Health Forms Deadline: Health Forms must be received in counseling and health services by August 15 for Fall semester enrollment and January 10 for Spring semester enrollment.

The meningococcal vaccine is required for all students living in the residence halls. Please read the information on meningococcal disease and vaccine [PDF]. Students may waive the vaccination by signing the meningitis vaccine waiver, although we recommend that you obtain the vaccination.

Return the completed forms to counseling and health services. Do not include forms meant for other university departments and offices.

If the Health and Immunization Form [PDF] has not been received by the second week of classes each semester, a hold will be placed on your registration.
